1 - I enclose ........ discs from an Amstrad PCW ......... model. They contain:

          eg Microsoft Word 97, Word 2003-SP3, MS Works XP, Appleworks;
          please don't just say 'Windows XP' or 'Vista', that's the Operating System!

3 - My computer can accept the results .....

Please now:

Clearly number your PCW discs then wrap each in paper kitchen towel
Do not send individual Perspex disc cases as these can shatter in the post
To keep within UK Large Letter thickness limit, pack in 2's in an A4 envelope
Put the disc pack between pieces of card or in an old (card) ribbon/disc box
Put this form with the discs in a padded envelope & mail to address below
If you want your original discs back, please include a self-addressed label
Note that the quantity discount is only applicable to error-free discs
